About the Fellowship


Message from the Fellowship Director

Welcome and thank you for expressing interest in population health fellowship training at Lancaster General Health. The combination of high-quality healthcare services and an institution-wide dedication to community care, make Lancaster General Health the perfect location to develop skills in population health.

The fellowship’s goal is to develop a pipeline of primary care providers with new skills in leadership as well as institute physician training focused on complex and special population management. Time in the fellowship is split between clinical work in Care Connections and work within our Accountable Care Organization, Lancaster General Health Community Care Collaborative. Interdisciplinary education – team development and modeling – is a critical part of the education.

The fellow may achieve training in a master’s level program such as the Masters of Healthcare Administration from a partner institution at The Pennsylvania College of Health Sciences. The fellow is also expected to teach and precept residents in the nationally-recognized Lancaster General Health Family Medicine Residency.

Care Connections began with a small pilot within the LG Health Family Medicine Residency. From 40 patients to more than 300, Care Connections has grown exponentially. At its core, Care Connections strives to empower patients and teach accountability. The team has scaled efforts to meet patient demand by using technological support such as MyLGHealth portal. Care Connections has also become an educational hub supporting the learnings of team-based care across the health system.

The aim of Care Connections is to better serve patients with three or more medical problems plus behavioral health issues, who have been admitted to the hospital multiple times in the past year. The program provides quality care with a customized experience, while lowering overall healthcare costs. Support for the medical team is provided by patient care navigators, social workers (including a Licensed Clinical Social Worker), a medical-legal attorney, hospital chaplains, care management nurses, a behavioral health psychologist, physicians, nurse practitioners, and a clinical pharmacist. Most importantly, Care Connections has been designed with our patients as integral team members.

Care Connections has received support from the County of Lancaster, the Commonwealth of Pennsylvania and multiple community stakeholders including the local United Way and won the prestigious Living the Vision Award of the Hospital & Healthsystem Association of Pennsylvania in 2015. Care Connections was also instrumental in LG Health being a finalist in the American Hospital Association Foster G. McGaw Prize, recognizing hospitals that have distinguished themselves through efforts to improve the health and well-being of everyone in their communities.

  • To develop new physician leadership for LG Health through active participation in Care Connections activities, coursework and attend/convene work groups
  • To develop a regional professional network for collaboration and exchange in new healthcare delivery systems and innovative approaches to population management
