  1. Discuss initiatives to prevent/reduce hospital readmissions for discharged atrial fibrillation patients.

  2. Discuss initiatives to prevent/reduce hospital readmissions for discharged heart failure patients.

  3. Review the CMS Million Hearts Campaign.

  4. Review signs and symptoms of peripheral vascular disease and current therapies.

  5. Review the current statistics, gender differences, clinical presentation for heart disease in women.

  6. Describe types of cardiac trauma, including treatment and outcomes.

  7. Review the relationship between sleep apnea and cardiovascular disease.

  8. Review inherited cardiomyopathy and the role of genetic testing.

  9. Review transplant program collaboration between Penn Medicine and LG Health.

  10. Review research program at LGH and how it impacts quality of care and healthcare  delivery.

From North (via PA Turnpike):
Exit PA Turnpike via Exit 286 toward US Rt. 222 South. Continue on US Rt. 222 South and merge onto US Rt. 30 East. Take the PA Rt. 23 East exit – New Holland Avenue/Pike. Turn right at the top of the exit ramp toward New Holland Avenue/Downtown Lancaster. Burle Business Park is approximately .3 miles on the left. From West (via PA Rt. 283 East/Rt. 30 East):
