Availability: Full-Time
Description: The Case Manager assumes responsibility and accountability for the coordination and implementation of case management strategies pursuant to the Case Management Plan. Plans and coordinates care of the patient from pre-hospitalization through discharge. Coordinates the discharge planning function in conjunction with the social worker, admissions coordinator and assists the DPCS with screening, investigation and problem solving as needed.
Requirements: Appropriate minimum degree for preferred licensure or certification or commensurate experience in a related healthcare field; Healthcare professional licensure preferred as Registered Nurse, LPN, Respiratory Therapist, Physical Therapist, Occupational Therapist or Social Worker; Minimum of 2 years social work or case management experience; acute/rehabilitation hospital experience preferred.



Availability: Full-Time, 2nd and 3rd Shift
Description: The Registered Nurse provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.  Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary.  Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate.  Participates in discharge planning process.
Requirements: Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program; Current state licensure as Registered Nurse; BLS certification (ACLS preferred).



Availability: Full-Time

Description: The Speech Language Pathologist (SLP) assumes responsibility and accountability for a designated group of patients and provides speech and language therapy evaluations, modalities and treatments in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality patient care.
Requirements: Certified by ASHA for Certificate of Clinical Competence in Speech/Language Pathology; Current Licensure as a Speech and Language Pathologist/Therapist in the State of PA; Current Basic Life Support/CPR Card



Availability: Full-Time, 2nd and 3rd Shift
Description: The Nursing Rehabilitation Technicians work under the direction of licensed nursing staff to provide comprehensive Rehab nursing care.  Assisting with: obtaining vital signs, patient transfers and transport, patient education, patient ADL's, therapy and working collaboratively with the Therapy Team and nursing team.
Requirements: Computer skills and strong interpersonal skills; ability to lift 50 pounds; current CPR certification; and high school education or GED.
