Transition from Pediatric to Adult

We believe in building long-term relationships with the families we serve. We also believe that every person should get age- and developmentally-appropriate healthcare. That’s why, as your children become adults, we are happy to help with the transition from pediatric to adult care. This transition usually takes place between the ages of 18 and 21, depending on his or her needs. We will work with you to address providers, readiness to make decisions, insurance issues and more. As your child assumes a greater role in his or her own healthcare, our goal is to connect him or her with the resources and specialists available at Lancaster General Health and to provide the support necessary to help your child choose well to be well.

Addressing special needs

This transition is important for every child, but especially for those with chronic illnesses such as asthma or diabetes, or physical challenges. It is also important for children with cognitive, emotional or developmental challenges. We can connect families with a wide range of community resources, including vocational training, job placement and day services.
