A Long-Term Approach for a Lifetime of Health

A Patient-Centered Medical Home is not a building, house, hospital, or home healthcare service, but rather a team-based healthcare approach that provides comprehensive, family-centered and coordinated care with the goal of obtaining maximized health outcomes.

Our practice as your medical home is a central resource for all of your child's healthcare needs. Your doctor/nurse practitioner and our staff include you and your child as the most important members of the healthcare team.

Coordinated, quality care.

A medical home streamlines access to all facets of the healthcare system. We coordinate all the service you need and act as your primary advocate. A medical home is designed to improve patient care while reducing healthcare costs. Medical homes consistently treat patients according to the best available scientific evidence and the principles defined by the largest primary care groups in the country, including the American College of Physicians, the Academy of Family Physicians and the Academy of Pediatrics. A practice must meet rigorous requirements to be recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance, a non-profit organization dedicated to improving healthcare quality.

Our responsibilities

As your medical home, we will provide to you:

  • Same-day appointments.
  • Quality care for your child when he or she is well or sick, and the promise to end each visit ensuring you have a clear understanding about expectations, treatment goals and future plans.
  • Information on pediatric specialists, home care, equipment and support services.
  • Educational materials to your family regarding your child's health because information can empower you and your child to improve his or her health.
  • Utilize electronic medical records to be able to quickly access information about your child's health so we can more effectively treat your child in a timely matter.
  • Ask you to use technology, such as MyLGHealth, which provides a more convenient way to communicate with our office and your healthcare provider.

We ask that you, as the most important part of the team, do the following:

  • Bring your child for well exams as recommended by the American Academy of Pediatrics.
  • Guide your children in making healthy decisions about their daily habits and lifestyle.
  • Remember to tell us about any care received between visits (for example, emergency room visits, urgent care and specialist appointments)
  • Bring a list of your child's medications to his/her appointment. If your child has asthma, please bring asthma medications and equipment to all of your child's appointments
  • Be sure to leave our office with an understanding of expectations, treatment goals and future plans.
  • Feel free to contact us with any questions or concerns for all health needs unless an emergency. If you have an emergency, call 911.
