A medical home means comprehensive care
From delivering babies to making nursing home visits to caring for hospitalized patients, we provide complete care for our patients. The National Committee for Quality Assurance has recognized our practice as a Level III Patient-Centered Medical Home. In addition to the care you receive from our board-certified  family doctors, a care manager works closely with you to make sure you have the information you need to choose well and be well. Our nurses follow up with you to make sure your health is on track. Plus, a social worker is available to remove any barriers - such as transportation problems or insurance issues - that might prevent you and your family from receiving the care you need.
Committed to your community
We take our commitment to community health seriously. We offer community outreach programs in Columbia Borough and provide sports physicals for Columbia School District’s athletic teams. Our offices in Columbia and Wrightsville offer evening and weekend hours, and providers are on call 24 hours a day, seven days a week.

We also offer access to MyLGHealth, a safe, secure online tool where you can make appointments, get referrals, request prescription refills and view test results at your convenience, 24 hours a day, seven days a week.
