Caring for You During Your Hospital Stay

Internal Medicine Hospital Specialists is pleased to direct or assist with your care while you are at Lancaster General Hospital. Our internal medicine physicians provide inpatient care in the role of hospitalists. Hospitalists are physicians who provide general medical care of hospitalized patients. Because they specialize in caring for hospitalized patients, they are very experienced in the unique needs patients have during a hospital stay. Since they spend their entire work day in the hospital, they are easily accessible by patients. Hospitalists are also in the hospital after hours and on weekends.

Our team includes nurse clinicians and physician assistants who greatly assist us in your care. They visit patients with us, help coordinate your hospital care, assist in communication with your family and are essential in planning your hospital discharge and follow-up after you return home.

Services Provided

  • We will coordinate your care during your hospitalization, working with many other members of the hospital team, including your nurses, social workers, therapists, pharmacists and consulting physicians.
  • We will arrange your hospital discharge or assist in coordinating your discharge if we are a consulting physician.
  • We will communicate with your family physician. He or she will receive a copy of our admission or consultation note as well as a copy of our discharge documentation. This provides a summary of your hospital care, lists your medical problems, discharge medications and plans for follow-up after discharge from the hospital. This is faxed to your doctor's office when you are discharged from the hospital, ensuring that your family doctor has crucial information for providing your ongoing care.
