Medical Home

In 2005, Roseville Pediatrics began implementation of patient-centered medical home. A patient-centered medical home is not a building, house, hospital, or home healthcare service, but rather an approach to providing comprehensive, family-centered and coordinated care.Our practice as your medical home is a central resource for all of your child's healthcare needs.  Your doctor/nurse practitioner and our staff include you and your child as the most important members of the health care team.

As your medical home:
We will provide quality care to your child when he or she is well or sick.
We will provide information on pediatric specialists, home care, equipment and support services.
We will provide educational materials to your family regarding your child's health because information can empower you and your child to improve his or her health.
We will help you plan your child's care and/or set goals for care.
We utilize electronic medical records to be able to quickly access information about your child's health so we can more effectively treat your child in a timely matter.
We ask you to use technology, such as MyLGHealth which provides a more convenient way to communicate with our office and your healthcare provider.
 
We ask you as a member of the medical home team to:
Bring your child for well exams as recommended by the American Academy of Pediatrics.
Keep your appointments and arrive 10 minutes early before your appointment time. Please let us know 24 hours in advance if you cannot make your appointment.
Take an active role in planning your child's care both at home as well as in our office.
Remember to tell us about any care received between visits (for example, emergency room visits, urgent care clinics and specialist appointments)
Bring a list of your child's medications to his/her appointment. If your child has asthma, please bring asthma medications and equipment to all of your child's appointments.
