Your vote can help Lancaster General Health advance its Palliative Care Services through support from LIVESTRONG’s Community Impact Project. LG Health and its partner Hospice & Community Care, founded as Hospice of Lancaster County, recently applied for grant funding from LIVESTRONG to gain Palliative Care Advanced Certification from the Joint Commission. This certification advances services to patients and families throughout the continuum of a serious illness.
Started in 2006, the program provides a dedicated team of certified hospice and palliative medicine physicians, nurse practitioner, social worker and chaplains who provide comprehensive palliative care throughout Lancaster General Hospital. Last year, the program served more than 1,000 patients and families, providing nearly 400 monthly visits to those needing pain and symptom management, advance-care planning, supportive counselling and end-of-life care.
LG Health is one of 70 other hospitals, cancer centers and community organizations competing in an online vote on LIVESTRONG.org that ends at 5 p.m. on March 28. Vote for the Certification for Advanced Palliative Care at http://vote.livestrong.org/regions/6/81-Lancaster-General-Hospital
