Neurological Day Treatment Program
Traumatic brain injury, stroke and other neurological disorders can significantly impair an individual’s ability to function. The team at LG Health can help you return to work or school and/or to live as independently as possible in the home or community. Our Neurological Day Treatment Program offers structured, comprehensive, flexible rehabilitation to help people with neurological disorders reach their full potential.
The goal: Removing barriers that block improvement
Staffed by a multidisciplinary team, the program takes a holistic, goal-driven approach to evaluation, treatment and education. A case manager, who is also a registered nurse, coordinates care and communicates with patient, family, physicians and third-party payers. Working with the patient and family, the team tailors a rehabilitation plan to address the individual’s specific functional goals through individual treatment sessions and therapeutic groups. The average length of participation is four to six weeks, but is determined by the patient’s needs and progress.

Participation in the program is by physician referral. The program is designed for patients who have sustained a brain injury or neurological dysfunction, who require only moderate assistance and who demonstrate potential for functional improvement. The nurse case manager conducts a pre-admission review to ensure appropriate admission, and the team meets every two weeks to discuss progress toward goals and adjust treatment schedules as necessary.
