Discharge Information

To better facilitate discharge, please provide the case manager with any and all information regarding the patient’s insurance, home situation, including family support and equipment he/she might already own. If you have any questions, or if anything is unclear, please do not hesitate to ask the nurse before departing from Lancaster General Hospital.


If your medical condition changes, or if you have any concerns regarding your medical condition, please contact your primary physician. If you do not have a primary physician, you may go to the Emergency Department. We have made every effort to be as thorough as possible to find all your injuries. Occasionally a patient may discover a new discomfort after discharge. If you note any new findings, please call our office to make an appointment (717) 544-5945 or talk to one of our providers.


Only take the medications as directed on the discharge instructions. If you are unsure or have questions about your medications, please call your doctor or nurse practitioner.



Tobacco Use

Tobacco in any form should be avoided. If you need help to stop smoking, contact your primary physician

Heart Failure

For adult patients with a diagnosis or history of heart failure:

  • Weigh yourself every morning before breakfast.

  • Limit the salt (sodium) in your diet to 2,000 mg a day.

  • Restrict fluid to two quarts or 64 oz a day.

  • Increase activity as tolerated.

Call your doctor if you:

  • Have increased swelling in your feet, ankles, hands or belly.

  • Have trouble sleeping.

  • Wake up feeling short of breath.

  • Need to sleep sitting up in a chair.
