Welcome to online appointment requests and pre-registration for Lancaster General Health. We are honored to provide healthcare services for you. Online appointment request and pre-registration allows you to make arrangements for your procedures when it is convenient for you. After completing the appropriate form, a representative will call to schedule your appointment.
You will also need to provide the following:
- The exact test listed on your order from your medical provider
- The diagnosis code(s)
- The insurance authorization or referral number(s)
