Dr. Wood

John Wood, MD, FAAFP

Medical Director
Dr John C. Wood M.D. is Chairman of Family and Community Medicine at Lancaster General Health. While a resident, he received the STFM resident teacher of the year award as well as serving as Chief Resident. Dr. Wood served as the inpatient medicine team lead for five years as Associate Director for the Residency. He is the Team Physician at J.P. McCaskey High School in Lancaster, PA and directs the musculoskeletal curriculum for the residency. As Chair, Dr. Wood serves as the Family Medicine eHealth Champion and focuses on transitions of care. Dr. Wood provides medical program direction and leadership to LGHealth’s Care Connections Clinic. His work with Care Connections focuses on innovative models of clinical delivery, patient risk stratification, transitions of care, novel payment methodologies and community/health system integration.

Kimberly Bahata, MBA, RN, CPHQ

Program Director & Director, Disease Management
Kimberly Bahata serves as the Director of Disease Management at Lancaster General Health. In this role she is responsible for the development and oversight of the disease management program targeting those diagnoses which correlate to cost and quality within the LG Health system. In January 2013, she co-led the journey to establish a comprehensive high risk program to effectively manage the highest need patient populations across all payer groups. She launched the Care Connections Clinic for high risk patients in August 2013.  Ms. Bahata joined the organization in 2008 and restructured the medical staff peer review program and directed the quality decision support analytic team. She has more than 15 years of quality, patient safety, and critical care nursing experience. She holds a Master’s Degree in Business Administration with a health care focus from Eastern University and is a Certified Professional of Healthcare Quality from NCQA.

Dr. Jeff Martin

Jeffrey Martin, MD, FAAFP

Lead Physician Extensivist
Dr. Martin is Associate Director of the Lancaster General Health Family Medicine Residency Program. He graduated from Goshen College in Indiana. He attended the University of Colorado, School of Medicine where he obtained his M.D. degree with honors in 1993. In 1997 he completed his family medicine residency at Lancaster General Hospital. During residency he received the Mead-Johnson Award for Family Practice Graduate Medicine. He directs the community medicine curriculum and has an interest in the genetic basis of disease. He is involved with several community endeavors including Medical Director at Vantage House, a long-term drug-rehab program for women and children, founding member of the steering committee for a County Health Department and Co-founder of the Lancaster County Lead Coalition. He began and directed Lancaster General's Superutilizer Project from 2011-2013 with a belief that patients with complex medical conditions and numerous hospitalizations needed help to break the cycle of repeat admissions. Along with his role as a faculty member in the Lancaster General Family Medicine Program, he provides clinical oversight and team leadership to LG’s Care Connections Clinic.

Wendell Kellum, MD

Physician
Wendell Kellum, MD is returning to Lancaster General Health eight years after his family practice residency training.  Dr. Kellum comes to the Care Connections team after a year-long fellowship focused on healthcare system redesign around the needs of high-utilizing patients with the Camden Coalition of Healthcare Providers and the Crozer-Keystone Family Medicine Residency. Prior to the fellowship, Dr. Kellum worked for seven years as a front-line family doctor with Esperanza Health Center, a faith-based community health center serving the predominately Latino population of North Philadelphia.  Dr. Kellum is enthusiastic about the mission of Care Connections and excited to join a team working on innovative ways that healthcare can support health and wellness for all patients, particularly those with complex conditions.

Jennifer Park, MD

Population Health Fellow
Dr. Park joins the Care Connections team as part of a Population Health Fellowship. She completed her family medicine residency training at Lancaster General Health, where she served as chief resident and was awarded the STFM Resident Teacher Award.  Prior to that, she completed undergraduate training at Duquesne University in Pittsburgh, PA, and medical school at Jefferson Medical College in Philadelphia, PA. Dr. Park has a strong interest in the relationship between psychosocial needs and chronic medical disease and is excited to work with a group dedicated to addressing these barriers. She is currently working toward a Master of Public Health degree at University of Pennsylvania and hopes to use this opportunity to help create relationships between community organizations and health systems to ensure the wellness of our population.  

Maribeth LeBreton, MSN, APN-BC

Maribeth LeBreton, DNP, CRNP, APRN-BC

Adult Nurse Practitioner
Maribeth LeBreton is a seasoned Nurse Practitioner who joins the Care Connections team after her work in cardiology, Community Case Management (management of patients with frequent hospitalizations at home to reduce hospitalization) Coumadin, insulin pump, pain management, palliative care, and with observation patients (chest pain, TIA, pneumonia). In her current role Maribeth assesses, treats and manages acute and chronic illnesses. Maribeth received her undergraduate degree from Villanova University and her Graduate and NP certificate from University of Pennsylvania. Maribeth completed her Doctor of Nursing Practice degree from Widener University. Maribeth is interested in the management of patients with complex medical and socioeconomic issues, She assess patient’s health literacy to provide the appropriate level of education to enable the patients  to overcome barriers so that they can navigate the health care system to receive high quality, exemplary chronic, acute and preventive care to meet their individual health care needs.

Jessica Rowland, MSN, CRNP

Nurse Practitioner
Jessica Rowland has joined Care Connections in 2014 as a Nurse Practitioner. Her previous position was as a Nurse Practitioner with a local nephrology practice, where she cared for mainly dialysis patients from the Lancaster County area. Prior to that, Jessica worked as an RN on a cancer and bone marrow transplant unit in Philadelphia. Jessica’s undergraduate education was done at The College of New Jersey and her NP training was done at the University of Pennsylvania. Jessica is passionate about the goals of Care Connections and is looking forward to being able to assist patients in attaining their health and wellness goals. In her free time, she enjoys horseback riding, camping, art, and nature.

Amber Jerauld, PharmD, BCPS

Amber Jerauld, PharmD, BCPS

Ambulatory Pharmacist Clinician
Amber Jerauld joined Care Connections in August of 2013 however was also a part of the “Superutilizer Pilot” that preceded the opening of Care Connections.  Amber received her Bachelor’s degree from the University of the Sciences in Philadelphia and completed her Pharmacy Residency at the Veterans affairs Hospital in Denver, CO and her Primary Care residency at Kaiser Permanente CO. Amber came to Lancaster General Health in 2010 as the Family Medicine Pharmacy Specialist and had inpatient and outpatient pharmacy responsibilities.  However over the past year, she has transitioned to working fully in the ambulatory care environment.  Amber has a passion for direct patient care and wants to help patients reach their medication goals and increase their medication knowledge

Audrey Martin, LSW

Clinical Social Worker
Audrey Martin is Care Connection’s own social worker. With over seven years of experience, Audrey provides patient assessments and individual/family counseling surrounding adjustment to illness and psychosocial barriers to health utilizing a strength-based, ecological systems approach. Audrey believes in patient empowerment and advocacy utilizing the patient’s identified goals of care. Audrey attended Messiah College for her undergraduate work and Roberts Wesleyan College for her Masters in Social Work degree. Audrey has been influential in Care Connections during the development phase and continues to help the program grow and flourish.

Case Manager, Office Nurse
Donna Kauffman joins Care Connections from the world of inpatient nursing where she has spent over 37 years on critical care, cardiology, and perioperative units. Donna received her RN at Lancaster General’s School of Nursing and her BSN from Messiah College. As the office nurse and care manager Donna assists with refills, medication concerns, inoculations, coordination of medical equipment, education, and coordinating care with all providers. Donna is thrilled to be able to help Care Connections and the patients in our program.

Laura Bowser, BSN, RN

Nurse Case Manager
As a nurse case manager, Laura works collaboratively with her patients and the Care Connections team to facilitate and advocate for the patients’ health care needs. Her role as a case manager is twofold in that she assists in discharge planning for hospitalized patients in addition to providing nursing services for patients in the Care Connections outpatient PCP office. Before joining the Care Connections team, Laura worked in various nursing positions, mostly in perioperative services, and has a passion for preventative health care. She received her Associates Degree in Nursing from Prince George’s Community College in Largo, MD and her Bachelor of Science in Nursing degree from Lancaster General College of Nursing.

Lee Ann Haas, Paramedic

Patient Care Navigator
Lee Ann Haas is Care Connections Paramedic. She has been an EMT-Paramedic for 30 years working 28 of those years for Lancaster EMS. Over the last 5 years Lee Ann has spent her time educating and instructing new EMT’s in Emergency Medicine. Most recently, Lee Ann has joined the LGH Care Connections team educating patients in how to care for themselves and their health problems. Lee Ann’s professional goal is quality patient care and commitment to the community.

Rick Canard, EMT

Richard Canard, EMT

Patient Care Navigator
Richard started in the health care field in 1974 working as an Emergency Room Technician and moving through the military and private industry. In 2010, Lancaster EMS hired Richard as a full-time BLS provider and CPR/First Aid Instructor and he was subsequently certified as an EMS Instructor with the PA Department of Health. Rich has been Lancaster EMS EMT of the Year for 2012, Star of Life Award 2013 recipient by the PA Ambulance Association and North Museum “What In the World” Career Program Volunteer of the Year 2013. Richard currently holds positions as Volunteer Coordinator and Community Outreach Coordinator with Lancaster EMS; and as Patient Care Navigator with Care Connections.

Patient Care Navigator
With over 13 years of experience working in a range of healthcare settings, Catherine shares knowledge and compassion for the diversity of patients Care Connections serves. As a Patient Care Navigator Catherine is focused on promoting wellness and self-care, while always seizing teachable moments within each one-to-one patient visit. Catherine is a firm believer that Care Connections will make a difference within the community by encouraging patients to become active participants in their own healthcare and wellness.

Richard Rush, BA, EMT-B

Patient Care Navigator
Richard Rush joins the Care Connections team with a five year background in the youth drug and alcohol prevention field. He received his bachelor's degree in Psychology/ Criminology from Millersville University, and is both a Nationally Registered and State Certified EMT-B. Richie is dedicated to the mission of Care Connections, and focuses on quality care and advocacy for all of its patients.

Jessica Shade, BS

Patient Care Navigator
With a BS in Exercise Science and minor in Gerontology from Slippery Rock University, Jessica joins Care Connections as a Patient Care Navigator. Jessica’s work experience includes clinical exercise physiology in cardiac and pulmonary rehabilitation, community health education and telephonic care management. Jessica has a passion for health education and empowering people to take charge of their health and manage chronic conditions.

Chad Wickenheiser

Lancaster County BHD Liaison with LGH
Chad Wickenheiser is a graduate of JP McCaskey High School.  In 2003, Chad deployed to Iraq as an Infantryman with the 101st Airborne/Air Assault Division.  After receiving a Bachelor’s degree in Social Work from Millersville University, Chad was hired as an Intensive Case Manager with Lancaster County BHDS.  Chad is currently enrolled at Millersville University in the Social Work graduate program.  Currently Chad assists the Care Connections Clinic by bridging hospital and community social services. Chad is dedicated to ensuring each patient is treated with dignity and respect.

Justin Cook

Justin Cook

Clinical Support Specialist
Justin Cook is a ten-year employee of Lancaster General Health and joined the Care Connections team at the outset of the program in 2013. Justin’s primary focus is Patient Engagement and he is often the first team member to meet with prospective patients.  Justin began his education in the field of Music Education but is planning to pursue a degree in Health Care Administration.  Justin brings over 15 years of inpatient and outpatient experience from several different facilities including Unit Support, Direct Patient Care, Customer Service, Care Management, and Customer Engagement.  Justin, who met his wife at LGH, is a dedicated to his family and is an avid music enthusiast.

Cindy Kuzma

Clinical Support Specialist 
Cindy Kuzma joins the Care Connections team as a Clinical Service Specialist.  After working on the inpatient unit 8 Lime as a Unit Clerk for several years , she transferred to the Radiation Oncology Department at the Suburban Outpatient Pavilion as a Patient Access Representative and was there during the opening of the ABBCI Cancer Clinic. Once the clinic was opened, Cindy remained with the Radiation Oncology Department as a Customer Service Specialist and had a varied role with patient care along with triaging phone calls and  scheduling and registration of patients.  She is excited to join the team at Care Connections and to have a more  active, hands-on role with caring for patients.  Cindy brings over 6 years of inpatient and outpatient experience including Unit Support, Direct Patient Care and Customer Service. .   Cindy strives to be an effective communicator and advocate for our patients, and to make each patient feel welcome, and is looking forward to getting to know and help each patient. 

Michele Wickenheiser

Clinical Service Specialist 
Michele joins the Care Connections Team as a Clinical Support Specialist. Michele's goal is to build on those patient relationships and aid in delivering preventative care. She is dedicated to providing patients with a trusting and supportive place where they can focus on strengthening healthy practices and well solutions. Michele has more than twenty years of experience providing care to patients at Lancaster General Health. She has worked with Roseville Pediatrics and more recently, within both the surgical and recovery phases at Lancaster General Hospital. When Michele is not working she enjoys boating, snowmobiling and spending time with her family.

 

Francesca Zola, BSW

Community Healthcare Worker 
Francesca Zola joined Care Connections to collaborate with the team in enhancing community and human well-being. In February 2012, Francesca began working for Lancaster General Health in the Emergency Department and developed an interest in working with trauma survivors. Upon receiving her bachelor’s degree in Social Work from Millersville University and working as a Social Work Intern at Downtown Family Medicine, Francesca became passionate about improving human rights and social and economic justice while advocating for consumer empowerment. Francesca is also completing her Master’s degree in Social Work and Emergency Management.

Yadira Santana

Community Healthcare Worker 
Yadira Santana comes to Care connections as a Community health worker with over 7 years of experience in the health care environment. Yadira was a med student at PUCMM in the Dominican Republic. Is a graduate of medical assistant program and will soon be continuing her education at the PA College of Health Sciences Nursing Degree Program. As an LGH employee Yadira has worked with in-house phlebotomy, lab support services and is now a proud member of the care connections team. Yadira believes that breaking down patients social economic issues will directly impact and improve our community’s health. Yadira hopes helping patients navigate the community resources will be key to helping them take control of their health and their lives. 

Laura Ramsey, BCC

Staff Chaplain 
Laura Ramsey is a Board Certified chaplain through the Association of Professional Chaplains. She attended Susquehanna University for her undergraduate work in music and religion and the Lutheran Theological Seminary at Gettysburg for her Master of Arts in Ministerial studies. Laura then completed a one year chaplain residency at the Milton S. Hershey Medical Center before accepting a staff chaplain position at LGH. Laura is currently the staff chaplain for the Women and Babies Hospital and also spends two mornings per week with Care Connections. Laura is honored to be one of the first chaplains working with the Care Connections team.  She is excited to see the ways in which the team is integrating patient’s spiritual needs as well as their physical and emotional needs in the overall plan of care.
 

Shaila Mercado, BA

Supervisor of Administrative Support Services/LEAP interpreter 
Shaila Mercado joins the Care Connections team after working in the laboratory at Lancaster General Hospital where she held the position of Lab Support Coordinator. She holds a Bachelor of Arts in Spanish from York College of Pennsylvania with a minor in Business Administration and International Business. She is fluent in both Spanish and Italian and is also an LG Health LEAP Interpreter. Shaila is passionate about patient care and is honored to work with a great team that is dedicated to improve the health and wellness of patients at Care Connections. In her free time, Shaila enjoys travelling, photography, hiking and reading.

Adam Wilikofsky, PhD

Adam Wilikofsky, PhD

Dr. Wilikofsky comes to the Care Connections team with almost three decades of experience in patient care and graduate-level education and supervision. After earning an undergraduate psychology degree at Yale University and both a master’s and doctorate in psychology at the University of North Carolina at Chapel Hill, he worked at Beth Israel Medical Center in New York City as a supervising psychologist and a member of their Pre-doctoral Psychology Internship faculty. Dr. Wilikofsky has been at Lancaster General Hospital for more than 25 years and serves as both a practicing clinical psychologist and full-time member of the Family Medicine Residency faculty. Dr. Wilikofsky maintains a longstanding interest in enhancing the collaboration between those who seek care and those who provide it.
