New Patient Information

Welcome new patients to Care Connections!!!

If you have been identified as a new patient for our program your primary care physician has already reached out to you to tell you a bit about what we offer. Below is the process for new patients:

  • Your PCP informs you of the Care Connections program during one of your office visits.
  • One of Care Connections Patient Care Navigators meets with you and your PCP to review the program and begin to explore health goals you may have while enrolled.
  • You will then attend an office appointment with our team to review your medical history & your prescriptions, and a home visit with our clinical counselor.
  • On a regular basis, determined by you and your team, you will have visits with our Social Workers, Patient Care Navigators, Chaplain, Clinical Counselor, Pharmacist and provider.
  • Care Connections will be your PCP for 3-9 months. After this, you will return to your home PCP for future care and wellness.

Appointment Information

If you need to reschedule or cancel your appointment, please notify us as soon as possible, or at least 24 hours in advance, by calling (717) 544-2273. We strive to provide easy access to appointments, many the same day. Our Patient Care Navigators are also able to meet you in your home if you prefer.


For office visits, check in at the front desk. Your insurance information, address, and phone number will be checked at each visit.


Care Connections is located on the first floor of Lancaster General Hospital. You will find our office on the corner next to the visitor elevators in the main building.

We view the patient as the leader of a multidisciplinary team who will define and direct personal goals for health care empowerment.  Motivational interviewing will provide the framework to identify patient goals and subsequently focus care team (including patient) on care management plan.


We will strive to create sustainable solutions to change that will equip patients with skills and resources for effective self-management of health care issues for the long-term.


We commit to advocating for individual needs of patients within the health care system, the local community and on state and federal levels.  We also commit to advocating for new delivery models of care to help persons with complex needs and will strive to educate health care providers at all levels of training and experience.
