Lancaster General Health is one of 45 Centers for Excellence selected for a $500,000 state grant to fund treatment of opioid addiction for people with Medicaid.
There were more than 3,500 opioid overdose deaths in Pennsylvania in 2015. Two-thirds of patients who are detoxed from opioids do not get linked to appropriate follow-up treatment and care.
The Pennsylvania Department of Human Services program helps ensure that people stay in treatment for follow-up care and are supported within their communities. Treatment is team-based and “whole person” focused, with the explicit goal of integrating behavioral health and primary care.
LG Health will utilize grant funding to create community-based care management teams for people who are abusing or addicted to opioids. The teams will work with local emergency services, inpatient and outpatient treatment providers to ensure that people are engaged in ongoing treatment. The teams also will address other barriers to recovery, such as housing, employment or family issues.
