We encourage patients and families to view the following videos before and during a stay at the Lancaster Rehabilitation Hospital. Our team hopes that by watching these videos, patients and families will have a better understanding of what to expect as part of this level of care, thereby decreasing anxiety and increasing satisfaction on the journey to recovery.

What to Bring For Your Hospital Stay
Use this checklist to help prepare for your stay:
- Comfortable day and night clothes (family is responsible for personal laundry)
- Safe, comfortable shoes
- Personal toiletries
- List of home medications
- All insurance cards
- Photo ID
- Copy of advanced directive, living will, power of attorney
You must sign and complete an authorization form to obtain a copy of your medical records. You may complete the authorization in one of two ways:

Charges for Copying Medical Records
- Lancaster Rehabilitation Hospital will provide 30 copied pages free of charge for a patient’s personal use. Requests that exceed this will be charged at the rate established by the Pennsylvania Department of Health.
- Medical records sent from Lancaster Rehabilitation Hospital to another health-care provider will be sent free of charge
