Our Medical Records Department is committed to making sure our patients' health information is accurate, accessible and secure. 

Medical Record Request

Patients enrolled in MyLGHealth, our secure online patient website, can access much of their health information from their computer or mobile device at any time. You can also request your medical record through MyLGHealth. If you have questions, please call 717-544-5911, option 2.

If you are not a MyLGHealth member or you are requesting your medical record for legal, social security disability, insurance underwriting, or transfer of care, you must complete an authorization form. The form is available below or at the Medical Record Services office on the first floor of Lancaster General Hospital, Monday - Friday, 8:30 am – 5 pm. If you have questions, please call 717-544-5911, option 1. We are happy to provide you with 10 copied pages of your medical record free of charge. Requests exceeding 10 pages are charged at a rate set by the Pennsylvania Department of Health. The charge will not exceed $40 for up to 100 pages.

Birth Certificate Request

Lancaster General Health is not authorized to provide copies of your child’s birth certificate. Copies can only be issued by the PA Department of Health Division of Vital Records
