Existing patients who want to see their current provider can schedule through MyLGHealth. To schedule with a specific physician, use Find a Doctor.

When scheduling below, you may need to identify if you are a new or existing patient. If you are not sure if you had an appointment, choose new patient

• New patients: Have not had an appointment with the practice in the past 3 years. 
• Existing patient: Have had an appointment with the practice in the past 3 years.

Primary Care In-Person Visits

Schedule an in-person appointment with a Family Medicine or Internal Medicine practice.

Primary Care Video Visits

Visit with a Family Medicine or Internal Medicine clinician using a video-enabled device like an iPhone, iPad, Android mobile phone or tablet.

Specialty Care

Lab Tests & Mammograms

Lab testing is for Blood Test or Urinalysis only. An order from your provider is required.


COVID-19 Testing

An order is required from your provider for any COVID-19 test. For more information about testing or how to get an order, please review our COVID-19 testing information

Testing is available 7 days a week. Sunday appointments are only available at the Downtown Pavilion and Suburban Pavilion Outpatient Labs.

Urgent Care

In-person and video visits are available for your urgent care needs. Video visits are conducted using the MyChart app and your MyLGHealth account. 

*Insurance plans vary. Please check with your insurance carrier regarding coverage limitations, pre-authorizations or referrals that may be required prior to your visit. Some insurances also require you to see only your designated primary care physician in order for services to be paid.
